PAYMENT/INSURANCE POLICY
At Loft Chiropractic, we are committed to providing the best chiropractic care while making the payment and insurance process as easy as possible. We accept many major health insurance plans, and we are available to assist you in understanding your benefits. We can provide you with the necessary documentation to seek direct reimbursement from your insurance company in the event our doctor is not a participating provider. Please note that deductibles and copayments apply, and we will ensure that you are informed about a financial responsibilities. We work with a variety of health insurance providers including Medicare, Worker's Compensation, No Fault, Independent Health, Univera, Highmark, Cigna, Aetna, Humana, United Healthcare, and others. Our team can help answer any questions you may have about using your insurance benefits for chiropractic services.

NEW PATIENT DOCUMENTS
In order to expedite your first visit, we ask that you have your patient intake documents filled out prior to your visit. When you arrive, give the documents to the front desk for review. Files are in PDF format and should be viewable in all web browsers for mobile devices and PC. If your forms are not completed ahead of time, please arrive 30 minutes prior to your appointment time to fill them out. You cay also call our office and request them to be mailed to you. If you need assistance filling them out, please call our office and let a staff member know.
